Baby Arizona Program
Application Assistance

Visit the Baby Arizona Web SiteThe Baby Arizona Program is a simple, faster way for pregnant women to start getting health care before the application process for AHCCCS health insurance is complete.

AHCCCS is part of the State of Arizona. AHCCCS provides health insurance to more than one-million Arizonans who need assistance getting medical services for themselves, their children, or other members of their family.

Who should apply for Baby Arizona?
Pregnant women who:
• Live in Arizona
• Have little or no income
• Are United States citizens or qualified non-citizens.

How do I start the application process?
1. Schedule an appointment for an ultrasound exam. At the time of the appointment, let a staff member know that you are interested in applying for the Baby Arizona program and you will be scheduled to return for the application process. Our staff members will tell you what you should bring with you for the Baby Arizona application process.

2. At your Baby Arizona visit, you will complete your application with the assistance of one of our staff members. When your application is complete and has been submitted, we will schedule you to see one of our Obstetricians. View profiles of our doctors (including which AHCCCS plans they participate in and which hospitals they use for deliveries).

3. When you complete you application, we will assist you in making an appointment to see one of our doctors. . The application process should be 20 working days from the time the complete application is received at DES. You may receive prenatal care sooner, depending on the doctor's schedule.

If you do not qualify for AHCCCS health insurance, the doctor should set-up a reasonable payment plan for your prenatal services and should continue to care for you and your baby.
